Inter View With Pharmacist

The purpose of this assignment is to interview a pharmacist to learn more about interprofessional collaboration between prescribers and pharmacists. If possible, volunteer to shadow the pharmacist. Discuss the following in your interview:

  1. The process of filling a prescription.
  2. Common reasons pharmacists call providers to clarify orders.
  3. What is on a proper prescription.
  4. Common omissions on prescriptions they receive.
  5. Medication errors pharmacists encounter.
  6. The prior authorization (par) process and common par medications.

Write a 500-1,000 word paper summarizing your experience and what you learned from the pharmacist you interviewed. Reflect on how this information will help your prescription writing. Discuss intercollaboration between pharmacists and prescribers.

You are required to cite three to five sources related to intercollaboration to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.

Overview/Description: 

The purpose of this assignment is to describe, evaluate and discuss application of a nursing grand or mid-range theory. This assignment also provides the learner an opportunity to connect theory and research to nursing phenomena. Learners will develop an 8-10 page paper (excluding the title page and references) using APA style to address the elements listed below.

Theory/Author Name and Background 

  1. Select a Grand or Mid-Range Theory that is appropriate to your practice setting.
  2. Describe the theorist’s background in detail and discuss how their experiences have impacted the theory development.
  3. Examine crucial references for the original and/or current work of the theorist and other authors writing about the selected theory.
  4. Identify the phenomenon of concern or problems addressed by the theory.

Theory Description

  1. Explain whether the theory uses deductive, inductive or retroductive reasoning. Provide evidence to support your conclusion.
  2. Describe the major concepts of the theory. How are they defined? (theoretically and/or operationally) Is the author consistent in the use of the concepts and other terms in the theory?
  3. Interpret how the concepts are defined. Implicitly or explicitly?
  4. Examine the relationships (propositions) among the major concepts.

Evaluation

  1. Identify explicit and implicit assumptions (values/beliefs) underlying the theory. On what assumptions does the theory build?
  2. Examine if the theory has a description of the four concepts of the nursing metaparadigm. If so, how are they explained in the theory? If the metapardigm is not explained, what elements do you see as relevant to the theory and why?

The purpose of this assignment is to practice the skills you have learned and demonstrate your ability to assess the thorax, lungs, and cardiovascular system. For this assignment you will need to videotape yourself examining the thorax, lungs and cardiovascular system on a volunteer of your choosing. This should take you between 30-45 minutes. When you are done you will document your findings. You will need to submit both your video and your documentation. Below are the required pieces in your assessment. 

Thorax:

  • Assess the patients for any signs of respiratory difficulty
  • Inspect the shape and movement of the chest
  • Asses for chest expansion
  • Palpate for tactile fremitus posteriorly
  • Percuss the posterior chest
  • Percuss for diaphragmatic excursion
  • Auscultate posterior breath sounds
  • Auscultate for egophony, bronchophony and pectoriloquy
  • Examine the anterior chest
  • Observe for any abnormalities (size, shape, bruising)
  • Auscultate for tactile fremitus anteriorly
  • Percuss the anterior chest
  • Auscultate anterior breath sounds

CV:

Overview: As a community of practice, your task for this week is to  collaborate with professionals across the health care system. With your  community of practice, in the Discussion, find a gap in care or social  determinant that often results in poor health care outcomes. You will  begin to take the lead in advocating for and collaborating with others  to improve the health care outcomes for populations at risk.

Practicum Discussion: During this week, you will identify a  population at risk in your community. This population will be the basis  for your Practicum Discussions and your individual presentations over  the next 6 weeks, as well as the focus of your final PowerPoint  presentation in Week 6. To review, a population is a group of  individuals who share a common environmental or personal characteristic,  such as obese individuals who are at risk for diabetes or  cardiovascular disease (populations at risk) or those individuals who  are otherwise healthy and could stay healthy if they do not develop  risky behaviors (populations of interest). An example of this is  teenagers who dont yet smoke but might consider it due to peer pressure  (Stanhope & Lancaster, 2016). Some of the topics you might consider  are vaccination compliance, obesity rates among children and adults,  teenage pregnancy, or infectious diseases such as Norwalk virus, genital  warts, or sexually transmitted diseases/infections. You might look also  at emerging public health problems such as Chagas or the Zika virus.  Some of the places you might consider looking for information to  substantiate and support your ideas about populations at risk in your  communities are your local health department, the CDC, and the many  evidence-based websites that the CDC supports, such as the CDC Wonder  (http://wonder.cdc.gov/). You may also review the work of other  community groups that focus on improving health care outcomes for your  community. You should begin to support your selection of population and  ideas about their health care problems through the use of health data  and scholarly literature.
